What Should a Safe GLP-1 Weight-Loss Program Include?

When choosing a GLP-1 weight-loss program, it helps to look beyond the prescription. You should understand who will monitor your treatment, how medication decisions are made, and what support is available throughout weight loss and maintenance.

A well-designed program should include an individualized medical evaluation, clear medication information, ongoing follow-up, nutrition and activity guidance, and a plan for long-term care.

At MD Wellness Center & Med Spa, these are the principles that guide our approach. Here is what to look for—and what patients can expect from our program.

1. A Thorough Medical Evaluation Before Treatment

Before prescribing, your clinician should assess whether a GLP-1 medication is appropriate for your health and goals.

At our clinic, Dr. Kesa oversees an evaluation that includes a detailed medical history, laboratory testing, and a physical examination. We review your current medications and supplements, weight-related conditions, previous weight-loss efforts, and personal goals.

We also consider relevant personal and family medical history, pregnancy plans, diabetes treatment, gastrointestinal conditions, kidney concerns, gallbladder disease, and any history of pancreatitis. These factors help guide medication selection and monitoring; they do not all automatically rule out treatment.

The starting point is an individual assessment of your needs.

2. A Clear Explanation of Medication Options

Before starting, you should understand what medication is being considered, why it may be appropriate, how it is taken, its expected benefits, common side effects, important warnings, and available alternatives.

We explain these options so you can make an informed decision with your physician. Together, we consider your medical condition, treatment goals, preferences, insurance coverage, and out-of-pocket cost.

Your treatment plan is individualized. Not every patient needs the same medication, dose, pace of dose adjustment, or length of treatment.

We also explain whether the medication is FDA-approved for your condition, how it will be obtained, and which pharmacy will dispense it. When appropriate, we discuss medications prescribed off-label for weight loss, including the reasons for considering them and their potential benefits and risks.

Off-label prescribing means using an FDA-approved medication outside its approved labeling. This is different from using a compounded medication.

Depending on your prescription, medication may be obtained through a retail pharmacy or manufacturer programs offered by companies such as Lilly or Novo Nordisk. When clinically appropriate and consistent with applicable requirements, compounded medications may be available through our office.

Your physician should explain these differences. The FDA recommends compounded drugs only when a patient’s medical needs cannot be met by an FDA-approved drug.

3. Individualized Dosing and Gradual Adjustment

Ask how your starting dose will be selected and when adjustments may be considered.

When initiating GLP-1 treatment, we generally begin with a low starting dose and adjust gradually according to the medication’s prescribing guidance and your individual circumstances. This gives us time to review appetite, food intake, weight trends, side effects, and overall response.

We do not rush dose increases simply to pursue faster weight loss. Dose decisions should account for both benefit and tolerability.

You should also receive clear instructions for administering your specific medication and handling missed doses.

4. Ongoing Follow-Up That Reviews More Than Weight

Before enrolling, find out how often you will be seen, what follow-up visits include, and how refills are handled.

At MD Wellness Center & Med Spa, follow-ups generally occur every 30–60 days, depending on your treatment and monitoring needs. During maintenance, visits may be spaced up to 90 days apart when clinically appropriate.

At follow-up visits, we review:

  • Weight trends and available body-composition estimates, including muscle mass, body fat, visceral fat, and body water.
  • Appetite, cravings, energy, sleep, and bowel habits.
  • Side effects or difficulty tolerating treatment.
  • Blood pressure and other relevant health measures.
  • Changes in medications, supplements, or medical conditions.
  • Nutrition, hydration, activity, and strength-preservation goals.

Body-composition measurements provide estimates that we interpret alongside your clinical progress. Laboratory testing is repeated when indicated.

If you are preparing to begin treatment, our guide to the first 90 days on GLP-1 therapy explains the early adjustment period.

5. Nutrition Guidance That Supports Health

A program should explain how you will maintain adequate nutrition as your appetite changes.

We discuss nutrition and physical activity at each visit. Our guidance addresses protein, fluids, fiber, vitamins, minerals, and overall food intake, with recommendations tailored to your medical needs and circumstances.

We consider your food preferences, culture, work schedule, and lifestyle when developing practical changes. We also review whether weight loss is occurring at an appropriate pace and whether reduced intake is affecting your energy, strength, or well-being.

For more detailed guidance, see our diet tips for patients taking GLP-1 medications.

6. Physical Activity and Muscle-Preservation Support

Ask how the program will help you stay active and support muscle preservation throughout weight loss.

We develop activity recommendations around your starting fitness level, health conditions, injuries, and goals. When appropriate, these include resistance or strength-focused activity alongside other movement you can sustain.

Across the different phases of treatment, our goal is to support fat loss while preserving as much muscle and physical function as possible.

Our article on preserving muscle during weight loss explores this topic further.

7. Clear Instructions About Side Effects and Getting Help

Before treatment begins, you should understand the common side effects and important warnings associated with your medication.

We review relevant medical history, discuss ways to manage common discomforts, and explain when symptoms need prompt assessment or emergency care.

Patients can contact our team with medication questions or concerns so we can review the situation and recommend next steps. You should understand the clinic’s contact arrangements, expected response times, and instructions for concerns outside office hours.

For a medical emergency, including at night or after hours, seek emergency care immediately rather than waiting for a message or callback.

8. A Plan for Evaluating Progress and Maintaining Results

Ask how your provider will decide whether treatment is helping and what happens after the initial weight-loss phase.

We assess weight trends, appetite, cravings, side effects, and other relevant health measures over an appropriate treatment period. A lack of noticeable change after a few days does not, by itself, determine whether treatment is effective.

If progress or tolerability is a concern, Dr. Kesa reviews whether adjustments or other options are appropriate.

During maintenance, we reassess which medications and doses remain necessary and effective. We aim to minimize unnecessary medication and may reduce doses when clinically appropriate. Some patients may benefit from continued treatment to maintain results.

The National Institute of Diabetes and Digestive and Kidney Diseases explains that treatment duration depends on benefit and side effects, and some patients may need ongoing medication.

Maintenance planning also includes continued nutrition and activity guidance, monitoring for weight regain, and follow-up appropriate to your needs. We consider energy, mobility, relevant metabolic markers, and sustainable habits alongside changes on the scale.
