Bra rolls, back fat, and love handles are related concerns, but they affect different areas of the body. Bra rolls and bra bulges usually develop along the upper back and bra line, while flanks—commonly called love handles—are located lower on the sides of the waist.
These concerns may result from localized fat, loose skin, or a combination of both. At MD Wellness Center & Med Spa, we provide personalized, non-surgical body-contouring plans based on the treatment area, amount of localized fat, skin firmness, and your goal, without surgery or downtime.
Back fat may appear across the upper back, around the shoulder blades, beneath the bra line, or closer to the waist. The thickness of the tissue and the amount of loose skin can differ considerably from one area to another.
Non-surgical treatment is intended for localized contour concerns rather than general weight loss or large-volume fat removal. Patients with broader changes in body weight may benefit from addressing their overall weight before deciding whether body contouring is appropriate.
A consultation allows us to examine the precise area and determine whether treatment is likely to produce a worthwhile improvement.
Flanks are located along the sides of the waist and lower back. Excess fat in this area is commonly called love handles. Although love handles may extend toward the back, they are different from upper-back bra rolls.
For patients with a measurable layer of localized flank fat, non-surgical fat reduction may help improve the contour of the waist. If weight loss has also left the skin less firm, a personalized plan may include skin tightening.
Treatment is best suited to contouring a specific area. It is not a replacement for weight loss.
Determining whether the concern involves localized fat, loose skin, or both is an important part of treatment planning. Although these concerns often occur together, the amount of fat and degree of skin laxity can vary between the bra line, upper or lower back, and flanks.
Localized fat generally feels like a thicker, pinchable layer beneath the skin. Loose skin may appear folded, crepey, or less firm. Many patients have a combination of both, so treating only one component may not provide the desired improvement.
Both Vanquish ME™ and Exilis Elite® can contribute to fat reduction and skin tightening, but they are used differently. Exilis Elite settings can be customized to focus on skin tightening or to address localized fat and skin laxity during the same treatment. Vanquish ME may be considered when treatment is needed across a broader area of localized fat, with improvement in skin firmness also possible.
During your consultation, we evaluate:
Localized fat vs. loose skin: Determine whether the primary concern is excess fat, skin laxity, or a combination of both.
Treatment area assessment: Evaluate the size, location, and thickness of the tissue being treated.
Most appropriate treatment: Decide whether Exilis Elite®, Vanquish ME™, or a combination is best suited to your goals.
Treatment focus: Determine whether skin tightening, fat reduction, or both should be prioritized.
Expected improvement: Discuss realistic outcomes based on your anatomy and treatment goals.
Treatment plan: Review the expected number of sessions, timeline, and overall investment.
Suitability for non-surgical treatment: Determine whether non-surgical body contouring is likely to achieve your desired results.
The appropriate treatment depends on what is causing the unwanted contour.
Exilis Elite uses controlled radiofrequency and ultrasound energy to heat targeted tissue. Treatment settings can be customized to focus on skin tightening or to address localized fat and skin laxity during the same session. It may help improve firmness, smooth folds, and refine the contour of the bra line, back, and flanks.

Vanquish ME uses radiofrequency energy to treat localized fat across broader areas of the flanks, lower back, and love handles without incisions or anesthesia. The controlled heating can also support improvement in skin firmness as the treated area becomes more contoured. It may be appropriate when the concern extends across a wider area rather than one small, isolated fold.

Z-Wave uses radial pulse therapy and may be incorporated as an adjunct to selected body-contouring treatments to help improve the appearance and texture of the treated area. It should not be presented as the primary method of eliminating back or flank fat.
Some patients may benefit from treatment focused primarily on skin tightening, while others may need both localized fat reduction and tightening. The recommended technology and settings depend on the treatment area, tissue thickness, degree of skin laxity, and the patient’s goals.
Treatment begins with a free body-contouring consultation. We evaluate the location and thickness of the tissue, the condition of the skin, your general health, and the improvement you hope to achieve before recommending a personalized plan.
Depending on the treatment area and technology selected:
Treatment series: A series of sessions may be recommended to achieve the desired outcome.
In-office procedure: Treatment is performed at our Indianapolis office without surgery.
During treatment: Patients may feel warmth, pressure, or a gentle pulsing sensation.
Minimal downtime: Most patients return to their normal daily activities afterward.
Gradual improvement: Results develop over time rather than appearing immediately.
Individual results: Improvement depends on the amount of localized fat, skin elasticity, treatment plan, and your body’s response.
Bra-line folds, back rolls, and love handles may be caused by localized fat, loose skin, or a combination of both. An in-person evaluation helps determine which concern is present and whether Exilis Elite®, Vanquish ME™, Z-Wave Pro, or a customized combination may be appropriate.

At MD Wellness Center & Med Spa, body-contouring recommendations are based on your anatomy, body composition, skin condition, and goals. We explain what non-surgical treatment can realistically accomplish, how many sessions may be needed, and whether the likely improvement makes practical and financial sense for you.
No. Bra rolls and bra bulges generally occur along the upper back and bra line. Flanks are located lower on the sides of the torso and are commonly called love handles. A patient may have concerns in both areas, but treatment may differ depending on the location, tissue thickness, amount of localized fat, and condition of the skin.
