Vanquish ME, CoolSculpting, and liposuction can all address localized fat, but they work in very different ways and should not be expected to produce identical results.
Vanquish ME uses contact-free radiofrequency energy to heat a broad treatment area. CoolSculpting uses controlled cooling through applicators placed directly on selected fat bulges. Liposuction is surgery that physically removes fat through small incisions.
The right approach depends on the location and thickness of the fat, the amount of change desired, skin elasticity, medical history, tolerance for recovery, and whether loose skin or cellulite is also present.
MD Wellness Center & Med Spa offers Vanquish ME, Exilis Elite, and Z-Wave treatments. We do not offer CoolSculpting or liposuction. This comparison is provided to help patients understand their options and set realistic expectations.
| Factor | Vanquish ME | CoolSculpting | Liposuction |
|---|---|---|---|
| Technology | Selective radiofrequency heating | Controlled cooling or cryolipolysis | Surgical fat removal |
| Invasive? | No | No | Yes |
| Contact with skin | Contact-free | Applicator contacts the skin; some applicators use suction | Cannula inserted through small incisions |
| Anesthesia | None | None | Local anesthesia, sedation, or general anesthesia, depending on the procedure |
| Best suited for | Broader areas of abdominal and flank fat | Selected fat bulges that fit the available applicators | Larger-volume or more precise fat removal |
| Treatment plan | Series of six sessions at MD Wellness | One or more cycles or sessions per area | Often one procedure |
| Results | Gradual fat reduction with potential skin tightening | Gradual, localized fat reduction | More substantial fat removal |
| Effect on skin | RF heating can support collagen remodeling and mild tightening | Primarily fat reduction; not a dedicated body skin-tightening treatment | Does not remove excess skin unless combined with skin-removal surgery |
| Recovery | Usually no interruption of normal activities | Usually limited, although temporary effects can occur | Days to weeks, depending on the procedure |
| Notable considerations | Hydration, correct positioning, screening, and appropriate settings | Prolonged sensory changes, cold injury, and rare PAH | Surgical, anesthesia, infection, contour, and recovery-related risks |

Vanquish ME non-surgical fat reduction uses contact-free Selective RF technology to heat fatty tissue across a broad treatment area.
The device is positioned above the body without touching, freezing, or suctioning the skin. Its radiofrequency field can treat the abdomen and flanks from side to side during a single session.
Controlled RF heating affects the targeted fat layer while also producing a thermal response in the surrounding tissue. This can support collagen remodeling and tightening of mildly loose skin while localized fat is gradually reduced.
Vanquish ME is generally performed as a series rather than as a single treatment. Results develop gradually as the body processes the affected fat cells.
Patients generally experience controlled warmth during treatment. Because the device remains above the body, there is no suction, freezing applicator, incision, or injection.
Most patients can return to work, exercise, and other usual activities immediately afterward. Temporary redness or warmth may occur, but significant downtime is not expected.
Patients are instructed to remain adequately hydrated before and after treatment. Occasionally, an area may temporarily feel firm or uneven. In our clinical experience, this settles with time, hydration, and massage when appropriate.
This temporary firmness is not the same as paradoxical adipose hyperplasia associated with cryolipolysis. It does not represent progressive enlargement of the treated fatty tissue and has not required surgical correction in our experience.
At MD Wellness Center, Vanquish ME is combined with Z-Wave acoustic-wave treatment as part of our body-contouring protocol.
Vanquish ME and Exilis Elite both use radiofrequency energy, but they deliver it differently.
Vanquish ME is contact-free and treats a broad area. Exilis Elite skin tightening and body contouring uses a contact-based handpiece with monopolar radiofrequency and adjustable cooling.
The provider can adjust the cooling and energy settings to control the depth at which tissue is heated:
This allows Exilis Elite treatment to be customized according to whether the primary concern is fat, loose skin, or both. At MD Wellness Center, Exilis Elite is also combined with Z-Wave acoustic-wave treatment.
CoolSculpting uses cryolipolysis, commonly called fat freezing. An applicator is placed directly on the treatment area. Some applicators use vacuum suction to draw tissue into the applicator, while others do not use suction.
Controlled cooling injures a portion of the fat cells within the selected treatment area. The body gradually clears the affected cells over the following weeks and months.
According to the manufacturer, CoolSculpting Elite is cleared for visible fat bulges in specific areas, including the abdomen, flanks, thighs, upper arms, back, bra area, beneath the buttocks, and beneath the chin or jaw.
The number of cycles and sessions depends on the size, number, and location of the areas being treated.
MD Wellness Center does not offer CoolSculpting.
During treatment, patients may experience pulling, pressure, intense cold, stinging, aching, cramping, or numbness. These sensations may decrease as the area becomes numb.
Temporary effects after treatment can include:
Most sensory changes resolve, but numbness can sometimes last for several weeks or longer. CoolSculpting’s published safety information reports cases of numbness that continued beyond 12 or 16 weeks during clinical studies.
When CoolSculpting is used beneath the chin or jaw, exposure of nearby nerves to cold can temporarily cause lower-lip weakness or tongue deviation.
Rarely, yes.
Paradoxical adipose hyperplasia, or PAH, is a complication associated with cryolipolysis. Instead of becoming smaller, the fatty tissue in the treated area gradually enlarges. The area may become firm and can develop the shape of the treatment applicator.
PAH usually appears two to five months after treatment. It is different from temporary post-treatment swelling and does not resolve on its own. Liposuction or another surgical procedure may be required to correct it.
CoolSculpting’s official safety information estimates PAH at approximately 1 in 3,000 treatment cycles, although reported estimates can vary. This figure refers to treatment cycles rather than individual patients.
Other uncommon risks listed in the manufacturer’s safety information include:
Patients considering cryolipolysis should discuss these risks and relevant medical conditions with a qualified healthcare provider. See the manufacturer’s CoolSculpting Elite safety information and the FDA’s guidance on non-invasive body-contouring technologies.
Liposuction is a surgical procedure that physically removes fat through a thin tube called a cannula. The cannula is inserted through small incisions, and suction is used to remove fat from selected areas.
Because fat is removed directly, liposuction can generally produce a larger and more immediate reduction than Vanquish ME or CoolSculpting.
Liposuction may be performed using local anesthesia, sedation, or general anesthesia. The type of anesthesia depends on the area, amount of fat being removed, surgical technique, and overall treatment plan.
MD Wellness Center does not perform liposuction. Patients seeking surgical fat removal should consult a qualified plastic surgeon.
Recovery depends on the treatment area, amount of fat removed, surgical technique, and individual healing.
Patients may need:
The American Society of Plastic Surgeons indicates that activity may be significantly limited during the first week. Some patients return to work during weeks two or three, depending on their occupation and the extent of surgery. Swelling may continue improving for six weeks or longer.
Potential risks include:
Read the American Society of Plastic Surgeons’ information about liposuction safety and liposuction recovery.
Liposuction generally produces the greatest fat reduction because it physically removes a larger amount of fat during surgery.
Vanquish ME and CoolSculpting produce smaller, gradual changes without incisions. The tradeoff is between the amount of improvement and the treatment experience:
None of these treatments is intended to produce significant overall weight loss or treat obesity.
Vanquish ME creates a radiofrequency field across a broad area. It can treat the abdomen and flanks from side to side without drawing individual fat bulges into separate applicators.
CoolSculpting treats areas according to the size and shape of the selected applicator. Multiple placements or cycles may be required when several areas or a larger surface must be treated.
Someone with a broad layer of abdominal and flank fat may prefer the coverage provided by Vanquish ME.
Vanquish ME’s RF heating can address localized fat while also supporting collagen remodeling and mild skin tightening.
CoolSculpting is primarily a fat-reduction treatment. Although CoolSculpting Elite has a lax-tissue indication for treatments beneath the chin, it should not generally be presented as a dedicated skin-tightening treatment for the abdomen or other body areas.
Liposuction removes fat but does not remove a significant amount of loose skin. If substantial hanging or folded skin is present, liposuction alone may not be sufficient. A tummy tuck or body-lift procedure may be required when actual skin removal is the goal.

Cellulite is different from localized fat. It involves the relationship among the skin, underlying fat, and fibrous connective bands that contribute to dimpling.
Vanquish ME, CoolSculpting, and liposuction are not primarily cellulite treatments. Liposuction may not improve cellulite and may make existing surface irregularities more noticeable in some patients.
At MD Wellness Center, Exilis Elite and Z-Wave may be incorporated into a treatment plan when skin laxity and uneven texture contribute to the appearance of cellulite.
Patients primarily concerned about dimpling should be evaluated according to tissue texture, skin laxity, fat distribution, and severity. Learn more about our cellulite treatments in Indianapolis.
Fat cells damaged by Vanquish ME or CoolSculpting are gradually cleared from the treated area. Fat cells physically removed through liposuction are also no longer present in that area.
However, none of these treatments prevents future weight gain. Remaining fat cells can enlarge, and body proportions may change because of:
Maintaining a relatively stable weight can help preserve the treatment result.
Vanquish ME generally involves the least interruption because it is contact-free and does not involve freezing, suction, incisions, or anesthesia. Patients typically return to their normal activities immediately afterward.
CoolSculpting also usually has limited downtime, but numbness, tenderness, swelling, bruising, pain, or firmness may occur. Sensory changes may sometimes persist for several weeks or longer.
Liposuction requires surgical recovery. Patients may need time away from work, compression garments, activity restrictions, incision care, and several weeks for swelling to improve.
The total cost depends on the treatment area, number of areas, required treatment sessions, and degree of correction.
When comparing costs, consider:
The least expensive treatment is not necessarily the best value if it cannot produce the change the patient expects.
A candidate for non-surgical fat reduction is generally someone who:
Someone seeking substantial overall weight loss should address that goal separately. A physician-guided medical weight-loss program may be more appropriate before cosmetic contouring is considered.
Neither treatment is automatically better for everyone.
Vanquish ME treats a broad area using contact-free radiofrequency energy and can support both fat reduction and mild skin tightening. CoolSculpting uses controlled cooling through applicators placed on selected fat bulges.
The appropriate approach depends on the amount and distribution of fat, skin elasticity, medical history, desired degree of change, and preferred treatment experience.
The technologies have different risk profiles.
PAH is specifically associated with cryolipolysis and is not an expected complication of Vanquish ME. CoolSculpting also carries risks related to cold exposure, including prolonged sensory changes and rare cold injury.
Vanquish ME requires appropriate medical screening, positioning, hydration, and device settings because it uses RF heating. Patients must be able to recognize and report excessive heat or discomfort during treatment.
Vanquish ME should not be expected to reproduce the amount of fat removal possible with liposuction.
Vanquish ME offers gradual fat reduction and potential skin tightening without incisions or surgical recovery. Liposuction produces a more substantial change but requires surgery and healing time.
Vanquish ME uses RF energy to heat targeted fatty tissue. The thermal response can also support collagen remodeling and mild tightening of the overlying skin.
No. Vanquish ME is a localized fat-reduction and body-contouring treatment, not a weight-loss procedure. Changes may be visible in the treated area without producing a significant change on the scale.
At MD Wellness Center & Med Spa, Vanquish ME is offered as a series of six treatment sessions. Completing the full series allows improvement to develop gradually as the body processes the affected fat cells.
The degree of improvement depends on the thickness of the fatty tissue, treatment area, skin condition, weight stability, and individual response. Additional treatment may be discussed after the results of the initial series are evaluated.
No. MD Wellness Center & Med Spa offers Vanquish ME, Exilis Elite, Z-Wave, and other non-surgical body-contouring options. We do not provide CoolSculpting or liposuction.
Choosing among Vanquish ME, CoolSculpting, and liposuction requires balancing the desired degree of fat reduction with treatment coverage, skin laxity, recovery, risks, and cost.
Vanquish ME may be considered for broader abdominal and flank fat when the patient prefers contact-free RF treatment and gradual improvement. Exilis Elite can be added when more targeted fat reduction, depth-controlled skin tightening, or both are needed. CoolSculpting may be available elsewhere for selected fat bulges that fit its applicators. Liposuction may be more appropriate when a patient wants substantially greater fat removal and accepts surgery and recovery.
At MD Wellness Center & Med Spa, we evaluate the location and thickness of the fat, skin elasticity, medical history, and expectations before recommending a treatment plan. If non-invasive treatment is unlikely to provide sufficient improvement, a surgical consultation may be more appropriate.
